I am interested about creating questions and answers log template, but I do not know the procedure of it.
Please help me by giving proper guidelines.

You can use abode flash to create the question answer template.
First of all download the adobe flash and install it.
Open the program and follow the following steps:Â
1. Create a new Flash document choosing the existing Quiz Template.
File > New > Templates tab > Quiz Category > Modern Photo Quiz_style2 Template.
2. A new window which contains drag, drop, fill in blanks, True false, hot spot, hot object, multiple choice etc will open.
3. Add the question and answer option.
4. To take a preview click on control and then click on test movie.
5. Finally save the quiz.
