There seems to be some problem with your phone.
I'd recommended you to restore your phone to factory default settings. Make sure you do a back-up of all the necessary things. This would take your phone to an out-of-the box state – same as what it was while taking it out of the box.
You can refer your phone manual to restore your phone to factory default settings.
