What was the real cause for the Microsoft Azure Outage?

I came to know about the widespread outage the cloud went through and affected about 20 services in most availability zones around the world. Is there a transparency of the cause of outage? What was the protocol followed when a change in configuration was made? Has Microsoft come out with the real cause of this Azure outage?

What was the real cause for the Microsoft Azure Outage?

Microsoft Azure, Microsoft’s cloud-based platform, experienced an outage that started around 5 PM PT of November 19, 2014 that lasted for about 11 hours. This incident affected many customers around the world not only in the United States. The outage impacted both third-party applications that were hosted like Azure Storage, Visual Studio, and Virtual Machines including Microsoft’s own products like Xbox Live and Windows Store.

The outage on Microsoft Azure was caused by performance update. The effect of the update was the failure for the front ends to receive more traffic which then caused other services created on top to also encounter issues.
