Please help me where can I find free incident form online

Hello techyv.com!

I have here an old Windows 98 PC, and I' am in need of medical free incident form that I want to be downloaded online. I' am afraid that my MS word were old as well, so I guess I need something that can support a 97-98 document type. So please if someone out here knew on where I could download it, please give me a link. Thanks!

Hi Alex,

I did a bit of searching on the internet and found a lot of Medical Incident Form Templates.

Luckily, most of the websites offer free downloads of the said templates.  You may get them here,

and Medical Incident Report Form Template Doc here. 

In addition, these are all done in .doc file extension, which should be compatible with your version of Microsoft Office.
