I need CSS templates for questionnaires

Hi there.  I would like to create a questionnaire for customer feedback on my website.  Can someone suggest CSS templates for questionnaires with Flash content?  Any help would be much appreciated.  Thanks in advance.

There are many ways to create a survey.

 

you can use Google Doc which a good tool actually to create templates.

Personally, I've used a tool on the web, called ""SurveyMonkey"", and it worked very well. you create your template, and the tool will generate a link that you will share with your mail list. Then to have access to answers, you can view them on your account SurveyMonkey.

Hello Elsietbrown,

                  If you want to make a survey of the CSS template then first you need is to see that which type of template you want to make. So first you have to specify the purpose of the template. After that you need to consider the audience among which you want to ask the questions. After that you can make the things easier to handle. You can ask about the ease and another aspect that have an impact on your HTML code. And also you have to ask about the abstraction level of the CSS file.
