Template on feedback to my software firm

I would like to have a page on my website in which users can enter feedback to my software firm and upon my approval, it gets added to the testimonial page. I would like to have a design on the feedback page, so please provide me few templates that will match my needs. Website is under construction, so a template with theme that support software will help me. Please do the needful.

Template on feedback to my software firm

Hi,

You would like to have a feedback page for your software firm in where user will be able to enter feedback & you can control this through the admin and if you will approve then it will be added into the testimonial page. Check the attachment to get an idea, you can share your testimonials or good feedback with your social networking accounts also. You can tell your friends posting them in your other profiles integrating with your website.
