Hallo Lyle Martin,

I think you are not able to use the custom form after creation because you have the option that prevents creation of the custom forms turned on. You will need to check that by and rectify in case it is the one preventing you from using the custom form by navigating to the following registry key:

HKEY_CURRENT_USERSoftwareMicrosoftOffice11.0Outlook

You should look for the DWORD named NoOutlookFormsDesigner, and if it has its value set to 1, reset it back to zero for you to be able to access the custom outlook forms. You should be careful not to mess up the registries for that may cause severe damage to your system, and in that case I will advise that you first backup the registry on a safe place on your computer.

For you instance, I am briefly describing the procedure to put out the form in the form library.

  • First of all you will make a new form.
  • Then you will require to click on Forms from the Tools Menu while you are working in design mode.
  • Now click on Publish Form, after this just click on Browse.
  • Now go to Go to Folder dialog box and click on subscriptions and click ok
  • After all this you will require to register for Subscribers as the Display name for the form. Remember it can also be the name of the form.
  • After this Click on Publish
