Convert Word Document To Fillable Form

I have to conduct a survey. I want a fillable form for that. Can you tell how I can convert a Word document to a fillable form?

Convert Word Document To Fillable Form

You will need Adobe Reader on your PC.

• Open the Word document you want to convert to a fillable form.

• Give print command or use Ctrl+P.

• Select Adobe PDF as the printer and click print. A dialog box appears for saving the file. Save it.

• Your file will open in Adobe Acrobat editing mode. Now you need to convert it to a fillable form.

Select Tools->Forms->Create.

• Few questions appear. Select use the existing file->use current document.

• Acrobat will try to recognize fillable fields. It will set decorative lines on the fields it finds fillable. Correct and set it manually.

• Select Preview to view the form or Edit to make changes.

• Save the form.
