SMS/MMS option not working properly for Nexus 5

I am using Nexus 5 on Tmobile. Recently I have noticed that half of the SMS and MMS messages I am supposed to send are not sent. Smilarly, in the receiving part also, many messages are not received by my phone. Why is this happening? Is there any kind of an update required here?

SMS/MMS option not working properly for Nexus 5

Hi Frank,
The Hangouts app is used to send SMS and MMS from the Nexus 5 phone.
There are two ways to send SMS through hangout app. The steps are mentioned below:
A) Add a phone number
1) Select the Hangout icon from the screen
2) Click the compose icon to open the new Hangout
3) Enter the phone number in the search box and select the 'send to SMS' option
4) The message will be sent to the respective number

B) Choose a contact from the conversation list
1) Select the person name from the list
2) Select the SMS option fro the dropdown list
3) There is an SMS icon with the contact profile picture
4) Choose the contact and type the message
5) Click 'Send' icon

The steps to send the MMS messages are as mentioned below:
1) On the right side of the message box, click camera icon to send the photo or video
2) Select the requires media option from the box displayed to the user
3) Enter the message to be sent to the user
4) Click the send icon and the message will be delivered to the recipient
