There is a really good application for this purpose.  It's called iForms, and you can download it from here:  http://www.limit-point.com/products/iform/.
 
This application lets you easily create html and css forms with no need for programming knowledge.  But, of course, you can view its html and css code if you want to learn how to make those forms manually.  Here is a screenshot of this application:
