Toast Titanium Cannot Write Medium – Incompatible Format error

I have been having a recurring error with Toast Titanium.  Almost every time I burn a DVD, somewhere during the burning process, it fails with this error.  Can anyone tell me what might be causing this?

Sense Key = ILLEGAL REQUEST

Sense Code = 0x30, 0x05

CANNOT WRITE MEDIUM – INCOMPATIBLE FORMAT
