Hahaha.. This is funny.. okay, first open your desired image and select filter then liquefy.. Now the fun part begins, you can make the brush size bigger or smaller by pressing the bracket keys "[ ]" Point your brush at the part you wish to stretch and start stretching.

The best method in doing this is Liquefy. You can also use the smudge tool but it is less effective.
