How long has the 24 x 24 pixel and 16 x 16 pix standard for Windows icons been in effect? Have those always been the standard? Or, was that standard implemented long after Windows became popular?

The recommendations of the icon sizes from Microsoft says that icons should include
·  48 x 48 pixels
·  32 x 32 pixels
·  16 x 16 pixels
·  But if you are using icons for making tool bar it is recommended using 16 x 16 pixels or use 24 x 24 icons.
